Do all Christian churches practice full communion as Jesus commanded?

Most churches do have communion on a regular basis, including either wine or a substitute (my church uses g juice) and bread (or a substitute). I've taken communion that was berry flavoured cordial before and many people simply use water for either cultural or dietary reasons- it makes no real difference. My pastor told of a church he visited once in a pacific island community where they drank water and ate a local variety of potato. But the intention is the same, and that's what matters :)
